I hereby authorize Hawaii Public Radio to initiate debit entries to my account indicated on the electronic form. I acknowledge that the origination of ACH transactions to my account must comply with the provisions of U.S. law.
This authorization is to remain in full force and effect until Hawaii Public Radio has received written notification from me of its termination in such time and in such manner as to afford Hawaii Public Radio a reasonable opportunity to act on it.
The recurring EFT payment will be withdrawn from my bank account and will be updated there on within two business days of the 15th of each month.
